Patient Flow Coordinator Cornwall Onward Care

We are looking to recruit an enthusiastic Patient Flow Coordinator within the Cornwall Onward Care Team, based in Derriford. This person requires previous experience in patient flow.
The aim is to achieve a collaborative approach to hospital discharge and ensure that patients only stay in hospital for the time required to complete their acute treatment, and that, where necessary, they experience timely and seamless transfer of care to another setting.
To assist with the efficient day-to-day running of the Patient Flow Team, providing administrative and clerical support to Nursing, Cornwall Council staff, and any personnel in Derriford, supporting the Onward Care Team.
Main duties of the job
* To ensure patients' needs are addressed based on the Care plan and coordinated in the most timely and effective way.
* To act as a link, liaising with other agencies and departments, to provide efficient and effective communication to expedite progress towards their onward care.
* Develop working relationships with ward teams to enable them to support and coordinate flow across the acute trust.
* In collaboration with clinical teams, health and social care agencies, the Patient Flow Coordinator will support the Integrated Transfer of Care Hub in planning and managing the daily flow through the Acute trusts.
* Will contribute to follow-up assessments, Care planning, identification of resources, and service delivery in order to facilitate the appropriate and timely discharge of patients.

Person Specification
Education/Qualification
* NVQ Level 4 qualification in relevant subject area or equivalent experience
* Experience of supervising/managing people
Skills and Aptitude
* An ability to act calmly in emergencies and to respond in a professional manner to stressful and challenging situations
* Able to prioritize workload of self and others
Knowledge and abilities
* Demonstrable knowledge of Patient Flow
* Ability to create innovative solutions to help support discharges
