Main duties of the job The Senior Discharge Coordinator holds specialist knowledge in this field and will work within the Integrated Discharge Team providing an expert and skilled community focused approach to discharge planning and assessment, embracing the principles of Discharge to Assess Model and providing a person-centred approach to care. As the Senior Discharge Coordinator they will be responsible to lead the team on a daily basis, prioritise daily workload, case manage the most complex discharge cases, coordinate daily capacity and support patient flow working with all key partners, whilst using their expert knowledge of community services to create seamless pathway of care from hospital to community and home. The successful applicant will be supported by the Community and Acute Hospital Service Managers in the Integrated Discharge teams service development, working with the team in clinical, health care and social care practice, providing knowledge, skills and experience of acute and community health care to our patients to achieve timely and safe discharge from hospital. The service operates 8am -6pm, 7 days a week, this post includes bank holidays and weekend working as part of the clinical team rota.
