Inviting applications to join our Integrated Discharge Team as a Facilitator at Southend Hospital!
We are looking for a new member to the team who is patient-focused and is keen to be involved in a dynamic environment where there is a constant drive for quality improvement to positively impact the outcomes of our patients.

Main duties of the job
1. Ensure the delivery of high quality, clinically effective and holistic care, through effective collaboration with multidisciplinary colleagues in everyday practice.
2. Assume accountability and responsibility for own decisions.
3. In conjunction with the IDT Assessors, provide support and training to healthcare assistants as appropriate to their experience.
4. Present a positive role model; strive to continuously improve standards of care whilst keeping up to date with innovations and change.
5. Contribute to the basic assessment and development of care plans for patients referred to the Integrated Discharge Team.
6. Provide planned advice, support and instruction to patients, external partners including care homes, care agencies, and equipment services, and where relevant their carers and other professionals, to promote understanding of the aims of their discharge plans.
7. Prioritise own caseload and provide facilitation to IDT Assessors when needed.
8. Promote the use of the Discharge Lounge to ensure patient flow and experience in discharge planning.
9. Educate and promote patients and carers to participate effectively in discharge planning where appropriate.
10. Gather core information to contribute to discharge plans for patients, including EDD’s and MFD dates and feedback to the team. Be competent in completing Assessment and Discharge Notifications in line with the Care Act.

* NVQ 3 or equivalent experience
* Evidence of continuing professional development
* GCSE/Equivalent
* Acute trust working
