Communicate regularly with patients to ensure that they are aware of upcoming appointments, diagnostic tests/investigations and operations and offer practical support with the arrangements. In conjunction with the Clinical Nurse Specialists (CNS) provide coordination of high-quality patient care through ongoing telephonic interactions with patients and proactive identification of needs such as WHO performance status, hospitalisation within this trust or other trusts, pregnancy, language barriers and arranging language interpreter during clinics to improve patient experience. Where appropriate, to be part of the pre- and post op calls to patients and to escalate any concerns to dedicated CNS To be responsible for meticulously monitoring diagnostic tracking list and document test progress, actively reducing delays and by maintaining close communication with both local and regional Cancer Multidisciplinary Teams, timely updates on test results and requirements will be ensured, optimizing patient care and treatment plans. To be responsible for supporting weekly cancer patient tracking meeting and to prepare, facilitate and record outcomes of each patient.
