About the Role: As an Advanced Practitioner in the Complex Care Team, you will be at the forefront of our efforts to support adults with the most complex needs. You will manage a diverse caseload, including high-risk and high-need cases, and provide specialist assessments and intensive interventions. Your expertise will be crucial in supporting adults through challenging situations, working with people with multiple long-term conditions and co-morbidities around drugs, alcohol, mental health, and homelessness; complex safeguarding, and court work, Key Responsibilities: Manage complex cases with a focus on holistic, strengths-based, and person-centred approaches Provide supervision, mentoring, and coaching to less experienced team members Lead on legal cases, court reports, and safeguarding investigations Develop and maintain positive relationships with partners and stakeholders Deputise for the Team Manager at professional meetings and MARACs etc as required What We Offer: A supportive and collaborative team environment.
