MSc Advanced Nursing Practice with aqualification with NMC

We are seeking a dedicated and compassionate Advanced NursePractitioner to join our healthcare team.
The ideal candidate will provide high-quality patient care;demonstrate clinical expertise; have an interest in elderly care; haveexperience in Palliative & Dementia care and experience in clinical triage.
Duties:
To help us deliver Enhanced Health in Care Homes
Conduct comprehensive physical assessments
Diagnose and treat common acute illnesses andinjuries
Manage chronic health conditions
Prescribe medications and treatments
Order and interpret diagnostic tests
Collaborate with physicians and other healthcareprofessionals
Qualifications:
Registered Nurse qualification with validNursing and Midwifery Council NMC registration
Completion of an accredited Nurse Practitionerprogramme
Proven experience as a Nurse Practitioner in aclinical setting
Excellent communication and interpersonal skills
Strong critical thinking and decision-making abilities
Hrs - 9.30am - 3.30pm - Monday - Friday

The post holder will:
Have an interest in elderly care to help us deliver the enhanced healthin care homes framework into our Residential and Nursing Homes within theboundaries of our General Practice moving away from traditional reactivemodels of care delivery towards proactive care that is centred on the needs ofthe individual residents, their families and care home staff.
have at least 1 years Advanced Clinical Practice experience
be responsible for contributing to the ongoing development of clinicalpractice, research and standards of care within the service, includingdevelopment of policies and guidelines and contribute to service development toensure patients have a seamless pathway.
be expected to practice autonomously, formulate clinical decisions andtreatment pans within local guidelines/policy to manage both acute and chronicillness.
We would welcome applications from practitioners with experience in thelong-term health needs of our aging population.
The post holder will require excellent communication skills which shouldinclude communicating clearly, sensitively and effectively with patients,families and professionals. Be able to signpost patients to the mostappropriate services and be enthusiastic to apply acquired knowledge andskills.

2 x G.P. Partners
2 x Advanced Nurse Practitioners
2 x Practice Nurses
1 x Health Care Assistant
Plus a full compliment of Administrative staff
We are part of the Stone & Eccleshall Primary Care Network
PCN Staff include:-
First Contact Practitioner; Urgent Care Practitioner; Team of Pharmacists; Pharmacy Technician; Social Prescriber; Mental Health Worker - all of which work across the 3 Practices within the PCN network.

Qualifications
* Advanced Clinical Practitioner must have an MSc in Advanced Clinical Practice/Clinical Masters.
* Qualified to Clinical Masters Level
