As a Senior Nurse Care Manager you will be central to a network of partners working across health and social care in Leeds who are committed to increasing integration for better services supporting people with learning disabilities.
You will manage a complex and diverse caseload to safeguard the welfare of individuals, families and carers. You will assess needs and manage risk, devise and implement support plans, commission services and evaluate outcomes, working with colleagues and partners to provide direct support to individuals and their carers.
You won't be doing this alone, you will be supported and guided by team managers, and in your turn play a key role in mentoring and advising other team members. Main duties of the job
About you
You will be qualified as:
* A Registered Learning Disability Nurse and registered with the Nursing and Midwifery Council
* An Approved Mentor/Practice Assessor
* Practice Educator Level 2 or equivalent, or undertake this on appointment (funded by us)
You will bring to the role:
* A minimum of 2 years post qualifying experience
* Knowledge and understanding of current learning disability practice and the role of a multi-disciplinary team, including statutory and voluntary agencies
* Skills in effective assessment, planning, intervention and evaluation of work in supporting people with learning disabilities
* Ability to build good working relationships with professional colleagues and partners, chair meetings and represent the council in a multi-agency environment and engage positively with individuals, families and carers
* Experience of using a person-centred approach, creativity in problem solving and reflective practice techniques
* Ability to lead, motivate, mentor and supervise staff, with a commitment to continuing professional development of self and others

* Appropriate professional qualification (Registered LD nurse or dual LD Nurse / Social worker qualification) and registered with the Nursing and Midwifery Council (NMC) upon commencement in post and maintain registration.
* Supported Learning in Practice/Approved Mentor/Practice Assessor Programme are essential on Appointment.
