EAST:MK Primary Care Network has exciting opportunity for a advanced paramedic into our practice team. The role is offered on a full-time basis, 37.5 hours per week over 5 days per week, between Monday and Saturday, on a rotation basis across the 4 surgeries - Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and The Grove Surgery that make up the PCN. The role of advanced paramedic is to support primary care to deliver high quality patient services as part of a multi-disciplinary team (MDT) across our practices. This may include home visiting, clinic working and patient caseload management. You must work within primary care as an autonomous, accountable paramedic providing holistic care for individuals including assessment, diagnosis, management, prescribing and treatment. The post holder will use advanced clinical and communication skills to provide education to patients, promoting self-care, a health and wellbeing approach to care and empower patients to make informed choices about their treatment. Responsibilities of the role will include: Assessing and treating patients in their own place of residence and those attending surgery. Triage and treat patients, making any necessary referrals to other members of the primary health care team and MDT and/or organise admission to hospital if required. Ensure clinical practice is safe and effective and remains within boundaries of competence and acknowledges practitioners own limitations. Advise patients on maintaining health and wellbeing, and refer to other team members as need arises. Communicate at all levels within the multidisciplinary team ensuring effective service is delivered. Maintain accurate, contemporaneous healthcare records appropriate to the consultation. Ensure evidence-based care is delivered at the highest standards ensuring delivery of high-quality patient care. Work within local policies and procedures. Participate in the audit process, evaluation and implementing plans and practice change to meet patient need. Contribute positively to the effectiveness and efficiently of the team and work colleagues. Take responsibility for safeguarding children and vulnerable adults and work with other team members to ensure they are protected. Essential: Educated to degree/diploma level in Paramedicine or equivalent experience. Registered with the Health and Care Professions Council (HCPC). Evidence of relevant CPD. Independent Medical Prescribing Qualification. Experience as a band 6, or equivalent. Advanced clinical practice skills. Experience of management of patients with common acute medical conditions. Experience of management of patients with long-term conditions. Ability to work well with colleagues and within a team. Ability to exercise sound judgements when faced with conflicting pressures. Ability to work flexibly to meet the needs of the primary care network. Highly motivated, with e nergy and enthusiasm. The ability to work under pressure and organise workload effectively. An enquiring and critical approach to work. Caring attitude to patients. Ability to communicate effectively with colleagues, patients, relatives and agencies. Full driving licence with exclusive access to a vehicle, as travel around the locality is a requirement of the role Desirable: Experience or working in a primary or urgent care setting. Experience of working in a GP practice MSc in Advanced Clinical Practice or equivalent Knowledge/use of primary care IT systems like SystmOne.
