Job responsibilities Main Duties and Responsibilities Provide and monitor care in the community setting: Provide compassionate, safe and effective care and support to people in a range of care settings. Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers. Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment. Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care. Recognise and report any situations, behaviours or errors that could result in poor care outcomes. Planning: Working alongside senior health professionals in planning the workload and completing tasks on Sys1/Malinko. Participating in co coordinating patients and workload. Communication To liaise as appropriate with members of the Adult Nursing team. Develop good working relationships and liaise as appropriate with parents/ carers, and other health, social or voluntary agencies. To communicate effectively with clients in complex situations. Communication skills of engagement, negotiation, motivation and explanation will be required to enable the management of potential barriers to communication. To be actively involved in the communication of complex information regarding clients needs and any other useful information to the Adult Nursing Team, and/or other appropriate medical services (such as GP) and members of the wider Adult Services team.
