Job overview
Working along side the community team with a special interest in palliative care and rehabilitation.
The team delivers care to patients which includes supporting with Activities of Daily Living, including personal care, catheter care, meal provision and support with nutrition and medication. End of life support and comfort. Care should be delivered in such a way to promote independence where possible and as part of an ongoing plan to support the patient to remain in their own home.
Care is delivered to ensure patients have a safe and timely discharge from hospital and prevent readmission or admission to hospital.
Main duties of the job
1. Where possible to promote independence with service users following the care plans put in place by therapy staff.
2. Supporting patients at the end of their life to ensure that care needs are met.
3. To deliver a high quality of personal care to a patient within their own home.
4. Documenting all visits on EMIS accurately and feeding back any concerns to your team lead/therapy staff.
5. You will need to have excellent communication skills.
6. You will be part of a very motivated team who deliver a high quality of care and rehabilitation to service users within their own home.
7. The team works 7 days per week, 365 days per year across between 08:00 and 22:00, staff may be expected to change their shifts pattern and work outside these hours with limited notice.
8. You must be a car driver with access to a car as the post will require you to travel between service users' homes.

Qualifications
* Basic Literacy and Numeracy skills
* Care Certificate or the willingness and ability to undertake (applicable for all non-registered support workers that provide health care)
* NVQ Level 2 in health and social care or equivalent experience
* Commitment to continued development of knowledge and skills.
Experience
* Working with Adults in a care environment.
* Experience of working within a team
* Experience of working within a rehabilitation setting.
* Experience of working in a community setting
Knowledge
* Knowledge of the NHS and Adult Social Care systems
* Knowledge of different approaches to care i.e. Rehab and Reablement
* Understanding of some common medical conditions which may affect older people.
Skills and Aptitudes
* Ability to work as a team member of a multi-disciplinary team.
* Able to exercise individual responsibility and judgement – be confident to work as a lone worker.
* Able to communicate effectively with colleagues, patients and carers – about a range of issues; using a range of techniques.
