Senior Physiotherapist - Intermediate Care
An exciting new opportunity for an experienced Physiotherapist has arisen to work as part of a therapy team at Carlton Court intermediate inpatient care unit based in Lowestoft.

The post holder will be expected to provide specialist clinical skills to manage a caseload of patients using evidence based practice in order to achieve their goals.
Main duties of the job
The post holder will provide a high standard of care as part of a multi-disciplinary team to patients within the intermediate care service and patients own homes. The post holder will be working as an autonomous practitioner applying a high level of clinical reasoning to ensure that individualised care is delivered safely.
The post holder will be responsible for delivering an Intermediate Care service for patients with highly complex needs within a ward-based setting however there may be a requirement to work across a wider community setting. The emphasis of the service will be reablement of patients in order to promote:
* Faster recovery from illness
* Prevention of unnecessary acute hospital admissions
* Prevention of premature admission to long term residential care
* Support for a timely discharge from hospital
Develop and maintain clinical skills in line with the roles job description and specific professional registration.
Provide individualised care plans for a complex patient caseload and ensuring that any delegated workload is completed by staff with the appropriate competencies.
Job responsibilities
* Accountable for care standards and the delivery of care promoting a culture of continuous improvement of clinical standards with particular emphasis on patient experience and key clinical outcomes.
* The post holder will adopt a case management approach to ensure that rehabilitation reablement is actioned and integration of care takes place.
* To assess, plan, implement and evaluate care and treatment, ensuring the highest quality of patient centred, goal directed care at all times, and with appropriate involvement and support of relatives and carers.
* To interpret and analyse clinical and non-clinical facts to form an accurate diagnosis and prognosis in a wide range of conditions, to recommend the best course of intervention and to develop comprehensive management strategies for the patients presenting condition.
* To enhance and develop the skills and dexterity, co-ordination and palpatory senses for assessment and manual treatment of patients.
* To refine treatment techniques in line with the most recent evidence of clinical effectiveness by evaluating and implementing key developments and promoting discussion within service.
* To provide complex information to the patients, clinicians and other agencies to assist in decision-making.
* To work with a defined caseload of patients with a broad range of health needs, using knowledge to carry out assessments.
* To formulate an individual management/treatment plan for each patient using clinical reasoning skills.
* To ensure own and other staffs interventions are evaluated and treatment is modified in order to measure progress and ensure effectiveness of intervention.
* To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals.
* To have working knowledge of equipment, minor adaptations and resources.
* Proactive participation in multidisciplinary team meetings.
* To complete clinical documentation and electronic data entry to a high professional standard.
* Promote confidence in the patient by means of appropriate professional standards of appearance, behaviour and attitudes.
* Act on unplanned situations using appropriate clinical judgement.
* Safeguard the confidentiality of all information regarding the patient and their family.

* To support the staff through the provision of clinical supervision, audit, appraisal, provision of support training and to be visible to staff, patients and relatives.
* To act as an effective role model to other staff, being clear about expectations and ensuring that safe patient care is central to all activities.
* To act as mentor to newly qualified, new staff and other learners.
* Provide clinical supervision/support to students and more junior members of staff.
* Actively participate in audit and research.
* Maintain and develop own clinical and leadership skills to support practice.
* Adhere to Health Care Professionals Council (HCPC) and The Chartered Society of Physiotherapists (CSP) Professional Standards and Conduct.
* Demonstrate a sound understanding of Clinical Governance and Risk Management.
* To develop and support evidence to meet the Care Quality Commission (CQC) fundamental standards.

Other Responsibilities
* The post holder will be expected to participate in a rota system of shift working to cover a 6-day a week service.
* To travel between ECCH sites for meetings and training.
