An exciting opportunity has arisen for a full-time, permanent CHC Nurse Assessor within the Adult Specialist Rehabilitation (ASR) division.
The post-holder will provide clinical input in the assessment and case management for CHC and will ensure that work programmes are planned, managed effectively and successfully delivered. Responsible for facilitating assessments and reviews against the national framework, within the agreed timescales, the successful candidate will promote high standards of communication and liaison with patients, their representatives and Health and Social Care colleagues.
Candidates should be experienced Registered Nurses with a working knowledge of the National Framework for NHS Continuing Healthcare and associated processes

The post-holder will provide leadership in implementation of the National Framework for Continuing Healthcare and NHS Funded Nursing Care for Adult & Specialist Rehabilitation Services.
