A rare opportunity has arisen for an enthusiastic individual to work within the area of End of Life. This post is to join the End of Life care team to work with patients who are in the last few days/hours of life. This is a very innovative team and is striving to take the Trust in the direction of achieving excellence in end of life care.
We are looking or individuals who are passionate about end of life care and wish to improve their skills and knowledge base within this specialised area. This will include assisting with the implementation of national guidance and initiatives, in particular the 5 priorities of care as outlined by the leadership alliance. It will also involve ongoing education and training trust wide around the principle of of end of life.
This is a challenging role and requires the individual to display empathy, genuineness and compassion to the patient and their loved ones at the end of their lives.

Qualification
Essential criteria
1. NMC registered
2. First level Degree (or working towards) in palliative care/end of life or equivalent experience
Desirable criteria
3. Advanced communication skills
4. Working towards Masters
5. Teaching Qualification
Experience
Essential criteria
6. Minimum of 3 years’ experience involvement of working with end of life patients and an understanding of palliative care needs.
7. Able to work independently, showing initiative and a desire to develop the service and initiate change.
Desirable criteria
8. Working within the acute setting and have experience of working with the End of life care team and patients
9. Demonstrate a knowledge and understanding of key national documents and guidance.
Skills
Essential criteria
10. Ability to work as part of a multidisciplinary team to ensure the patient pathway is seamless
11. Excellent interpersonal skills demonstrating evidence to guide and support others
12. Ability to manage a caseload of patients maximising time management and prioritising work load
Desirable criteria
13. Demonstrates clear commitment to the team approach; exchanging ideas and providing support to colleagues
