Role Summary
The role of the Seaton Hospice at Home Team is to provide specialist 24-hour support to patients with palliative care needs within the Seaton area. The role combines clinical skills with emotional and psychological support to provide a holistic, patient-centred service based on the individual needs of each patient. Our aim is to enable patients to remain in their preferred place of care and to have the best quality of life possible.

Are you a dedicated nurse committed to providing the highest standard of care? Do you have an interest in palliative care?
We are seeking a motivated nurse eager to develop their knowledge, skills, and experience working with patients with palliative needs. You will have opportunities for learning through clinical reflective practice and professional development, supported by the Seaton Hospice at Home team and the palliative care nurse team manager.
This is an exciting chance to join a professional, specialist palliative care team working alongside a multidisciplinary team of health and social care colleagues in the community and hospital sectors.
The Seaton Hospice at Home model integrates community services into a cohesive “one-team” approach to provide optimal care for patients diagnosed as palliative by their GP, regardless of diagnosis or complexity, guided by individual needs and wishes. The service ensures continuity of care, enabling patients to die in their preferred place, often at home.
The service is managed by the Royal Devon University Healthcare NHS Foundation Trust and funded by the Seaton League of Friends. It provides specialist palliative care 7 days a week, year-round, with shifts covering day and overnight on-call duties to respond to patients' needs, providing end-of-life care and support, and avoiding unnecessary hospital admissions.
Working Pattern: hours per week, including overnight on-call shifts.
