Main duties of the job
As part of the PrioryMedical Group digital team your role will primarily be using non-digitalcommunication tools to co-ordinate patient care to ensure our non-digitalpatients are not excluded from care co-ordination. The work will involvecontacting patients on the phone, processing patient responses, and maintainingthe practices digital systems and tools.
There will be a dailyrequirement to work with the rota, digital and patient central team to confirmthe clinic capacity available for urgent care which can then be used toco-ordinate care for patients, including the most vulnerable and complex.
There is an opportunity towork remotely from home as required, you will be provided with a laptop tofulfil your work requirements.

Job responsibilities
Main Duties
Provides co-ordination and navigation for patients through the health and care systems
co-ordination of clinic sessions to support complex and vulnerable patients or those who are non-digitally enabled to access care
ii.Undertaking outbound calls to identified non-digitally enabled patients who need a phone call or letter invite for appointments to ensure digital inclusivity and support their long-term condition management
iii.Booking patient appointments into services provided by health care professionals across PMG
iv.Supports patients in preparing for follow-up with provision of information on what they should expect when they attend for their appointment
data on the patient cohorts supported through outbound calls to support informing data dashboards for the digital team lead.
vi.Documenting submitted data against protocols, filing in the patient record and appropriately signposting for follow up care.
work with wider PMG teams to continually improve and develop the non-digital care co-ordination offer to patients.
