Main duties of the job

As a Senior ClinicalPharmacist, you will act within your professional boundaries to provide visibleleadership and operationalmanagement to the Medicines Optimisation Team. You will assist with the training and performance management of the MedicinesOptimisation Team.
As an autonomous practitioner you are responsible for thecare delivered, demonstrating critical thinking and skills in clinicaldecision-making in the management of patients. You will work collaboratively tomeet the needs of patients and the practice at all sites or establishments,ensuring the highest standards of care, supporting the delivery of policy andprocedures.
You must work at all times within your own competence anduse your own judgement, knowledge, and common sense in your day-to-dayactivities.

Job responsibilities
Clinical Pharmacy responsibilities
Your duties and responsibilities may include any or all of the items in this job description which may be varied from time to time under the direction of the Director of Prescribing or the Medical Director.
To adhere to the GPhC code of ethics, observing and applying all professional, ethical and legal obligations.

To undertake annual clinical medication reviews.

To ensure all major MHRA drug alerts, where appropriate and within your level of competence are acted upon within the practice.
Follow local and national Infection Control guidelines.
Demonstrate awareness of statutory and local child protection procedures, including systems of referral.
Contribute to achievement of the highest possible quality standards such as Quality and Outcomes Framework and other agreed Key Performance Indicator targets.
To facilitate and manage the development and delivery of a Practice Prescribing and Medicines Management Plan and the delivery of patient services as determined by Practice policy and local and national guidance.
To support the development of a practice based Medicines Management and Prescribing Team, including delivery of training, in order to maximise cost-effective prescribing and to improve the quality of patient care.
To promote high quality, cost effective and evidence based use of medicines within the practice.
To improve understanding of the principles of medicine optimisation.
To run specialist clinics for patients with long term conditions.
To ensure that all dispensing errors and near misses are logged in accordance with the companies procedures and to undertake any activity necessary to rectify or prevent errors.
To liaise with doctors about prescriptions to ensure accuracy and prevent risk.
To keep up-to-date with current pharmacy practice, new drugs and their uses for the purpose of continuing professional development.
To promote good practice in line with therapeutic developments.
To maintain computerised records in accordance with data protection principles to ensure patient confidentiality.
To counsel and advise patients on the safe and appropriate use of medicines.
To provide professional advice and guidance on medicines related queries and issues to the wider practice team and patients.
To advise and assist with all the above as required for Haxby Group and other associated businesses.
To provide regular support and feedback to the practice on prescribing action plans.
To update and maintain accurate patient medication records on the practices clinical computer systems including advice given and action taken.
To advise the Primary Health Care Team on the safe and secure handling of controlled drugs and other medicines, ensuring compliance with medicines legislation.
Undertake additional tasks as required within your general role as Practice Pharmacist.
You will develop, implement, and adopt standard operating procedures and processes to avoid risk and to enable the team to work more efficiently and proactively, standardising good practice across the business.
You will have an oversight of the team workload and highlight any areas of concern to the managers.
You will be responsible for representing the teams at a group level, working with other managers to ensure the requirements of the team are met.
You will have authority to divert internal and external resources to other locations if the workload requires.
You will be responsible for liaising with external contactors regarding hours, deployment of staff, work completed, and manage problems with support from the prescribing lead GP.
Work with the Seniors and Director of Prescribing to ensure sufficient staff of appropriate ability, quality and skill-mix are available to meet current and future service delivery. You will highlight the areas that are impeding the work of medicines optimisation, in particular manpower, staff training and experience, access to technology, and external resource support, and work to resolve these. You will be aware of relevant CQC, QOF, PCN and any other targets that need to be met, their timeframes and what is required to fulfil them. You will ensure enough resources from the practice are in place to achieve these targets.
Managerial and leadership responsibilities
You will act as line manager to Clinical Pharmacist(s) in the MOT where agreed on an individual basis. You will develop the skills to confidently lead on clinical projects to support patient care, both within the practice, and where integrated with external organisations as directed by the practice. Ensure the principles of infection control and local/national policies and guidelines are applied throughout the practice, monitoring and implementing changes as required. Assist in the management of complaints or significant events relevant to the Medicines Optimisation team, conduct investigations and ensure that actions arising out of investigations are implemented. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and external good practice. Support the development of others in order to maximise potential.
