A vacancy has arisen within our Treatment Room Team. We are taking this opportunity to reassess the needs of the practice, and whilst we are seeking a qualified nurse, ideally experienced in primary care, we would also be keen to speak to any Health Care Assistants or Phlebotomists who are looking for a new challenge. We are mindful that for various reasons, not least work-life balance, not everyone wishes to work full-time, so we would be willing to consider two or more part-time posts, possibly with varying skillsets.
Main duties of the job
Working as a member of the practice multidisciplinary team, the post holder(s) will ensure that general nursing services are delivered effectively to the patient population.
The successful candidate(s) will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, well women and well man clinics, venepuncture, wound care and ECGs as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure. There will also be a role caring for our frail patients, both in the surgery and in their own homes.
Painswick Surgery will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.
About us
We are a 3 partner General Practice with 4,800 patients located in the village of Painswick. We are part of the Stroud Cotswold Primary Care Network (SCPCN) with 4 other practices which allows us to combine our efforts to work on bigger projects (e.g., the delivery of the Covid-19 vaccination programme). We also have 2 salaried GPs working with us as well as 2 Practice Nurses and a Health Care Assistant, Clinical Pharmacist, a Pharmacy Technician and Social Prescribers. We are also a training practice for qualified doctors who wish to work as GPs.
Job responsibilities
The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
1. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice.
2. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
3. To develop, implement and embed health promotion and wellbeing programmes.
4. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing with a special interest and experience in managing patients with diabetes.
5. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
6. To provide routine nursing care to patients, to include venepuncture, wound care and ECGs.
7. As required, to diagnose and manage acute and chronic conditions.
8. To prescribe medication for patients in accordance with PGDs and own scope of practice.
9. To review patient medications to enhance compliance.
10. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages.
11. To co-ordinate the provision of travel medicine services.
12. To request pathology services and process pathology results as required.
13. To participate in the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
14. To maintain accurate clinical records in conjunction with extant legislation.
15. To ensure read codes are used effectively by all members of the nursing team.
16. To ensure the appropriate maintenance and use of chronic disease registers.
17. To develop, implement, embed and participate in well woman clinics.
18. To develop, implement, embed and participate in well man clinics.
19. To ensure compliance with the practice chaperone policy.
20. To assist GPs with minor surgery when required.
21. To prioritise health issues and intervene appropriately.
22. To support the team in dealing with clinical emergencies.
23. To recognise, assess and refer patients presenting with mental health needs.
24. To implement vaccination programmes for adults and children.
25. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).
26. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
27. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
28. To support the clinical team with all safeguarding matters, in accordance with local and national policies.
29. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
30. To deliver opportunistic health promotion where appropriate.
31. Ensure the robust monitoring of fridges and both refrigerated and ambient stock, for both temperature and expiry dates, and adhere to the strict policies for these and other monitoring tasks.
In addition to the primary responsibilities, the Practice Nurse may be requested to:
1. Develop and review audit protocols and process for the practice, effectively utilising the audit cycle.
2. Develop, review and implement nursing protocols in conjunction with the partners.
3. Support all members of the nursing team, providing guidance when necessary, conducting appraisals and training needs analyses as required.
4. Participate in local initiatives to enhance service delivery and patient care.
5. Support and participate in shared learning within the practice in order to improve patient care.
6. Continually review clinical practices, responding to national policies and initiatives where appropriate.
7. Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA).
8. Coordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs.
9. Lead the planning and implementation of changes within the nursing team, enhancing the provision of services to patients.
10. Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation.
