We have an exciting opportunity for an enthusiastic and self-motivated member of the care team to work as a Youth and Transition Worker to support the young people with life limiting conditions at Helen & Douglas House. This is a part time 22.5 hours per week 1-year fixed term maternity cover and the salary ranges between £32,656 - £39,303 per year pro rata dependent on experience. Interviews are scheduled to take place on the 12th March 2025.
As part of this exciting role, you will support our young people with their lives, enabling them to be the best they can be and to achieve the best out of their life. You will achieve this by creating supportive, positive, and trusted relationships. This will enable our young people to find out what is important to them, and what they would like to achieve. You will work as part of a team supporting the delivery of the hospices Community and Family Support Services team and will be responsible for managing a caseload of young people aged 13-19 across various locations within the 6 counties of the catchment area.
We are looking for someone with knowledge in youth and transition work to support the young people and their families, offering information and advice, signposting, and referring where appropriate, and undertaking youth work activities in various settings.
