Job summary Salford Care Organisation is part of the Greater Manchester Kidney Care Network, providing Renal Services to patients living in the North & West Sector of Greater Manchester. An exiting opportunity within renal services Transplant Team for a 12 month secondment to care for a growing post renal transplant population of 750 patients. This role is ideal for an experienced Staff Nurse or Band 6 Nurse who is looking to move into a challenging and rewarding role within the team. Main duties of the job This is an interesting and varied role that will provide a good leadership development opportunity for the right candidate. The ideal candidate would have some renal experience and be able to transfer skills into this specialised role caring for renal patients that have received a kidney transplant. You will be expected to provide a high standard of care to this complex patient group, by supporting them at their outpatient visits, providing nurse led clinics both at Salford Royal and within the wider catchment area at outreach clinics, assessment and triage of ongoing and complicated associated conditions and being at the forefront of their long term care. You will have a major role in educating and supporting both healthcare professional, patients and significant others with regards to the patient journey and with the ultimate aim of enhancing and maintaining quality of life. You will be a proven team player with post registration renal experience at Band 5. You will bring evidence of multi-disciplinary and multi-agency partnership working, and the ability to both influence and embrace change. Well-developed communication skills are essential. Non-medical prescribing qualification is desirable but not essential as training will be available to support the role. A car driver is essential to enable the running of outreach transplant clinics in Oldham, Bolton and Wigan.
