Join our friendly, supportive team as a qualified *Occupational Therapist* with HCPC registration. You’ll conduct home visits to lead holistic assessments, plan and review interventions. You’ll provide support across our multi-disciplinary Reablement Service, a strengths-based short-term service helping the people we care for maintain or regain independence in daily activities.
*What you’ll be doing:*
* Leading on the assessment, planning and reviewing of complex case work.
* Conducting person-centred functional analysis and interventions to help patients maintain or regain independence in daily activities.
* Collaborating with a diverse team, including Occupational Therapists, practitioners, carers, sensory staff and business support.
* Working independently, providing expertise and support to staff, informed by social care legislation and clinical knowledge.
* Our service operates seven days a week and there will be a requirement to work weekends on a rotational 1:8 basis.
*What we’re looking for:*
* Occupational Therapy qualification and professional registration with HCPC.
* Experience in assessment, planning and reviewing of case work.
* Ability to work independently and collaboratively, escalating cases as required.
* Commitment to demonstrating our values and behaviours.
* An understanding of the statutory duties of local government in social care and the social care model.
