To undertake the comprehensive assessment of patients with neurological conditions, living in the community of west Norfolk, whose needs are complex and may have multiple health/social problems. Using investigative and analytical skills to assess and to formulate individualised management, through mutually agreed goals and treatment plans, using clinical reasoning, and utilising a wide range of treatment skills and options to develop a specialised programme of rehabilitation. To have clinical responsibility for a caseload of patients, and to organise caseload using management skills, effectively and efficiently prioritising according to risk category. Act as a resource on issues of neurological Physiotherapy practice to all colleagues within the Community Neurology Service, and to the wider integrated teams. To provide specialist advice, teaching and training to other health and social care members of the wider multidisciplinary team regarding specialist neurological physiotherapy. To train and supervise assistant practitioner/therapy assistant staff on a daily basis, and students where relevant, on matters of specialist physiotherapy practice. To be professionally and legally responsible and accountable for all aspects of your own work including management of patients within your care. To ensure high standards of clinical care for patients under your management, and support more junior staff to do likewise. To be able to communicate effectively with patients and carers to maximise potential and ensure understanding of condition and rehabilitation process. There are many aspects of communication, which may be required with a variety of patients, e.g. persuasion, explanation, motivation, empathy and defusing sensitive and stressful situations. There will also be some barriers to effective communication, e.g. loss of hearing, speech and language deficit, confusion/dementia, pain and anxiety/fear. To be able to communicate effectively with other health or social care professionals in order to facilitate a multidisciplinary approach to the management of patients with Neurological Conditions in the community. To undertake appropriate training to allow working across the professional boundaries of basic OT practice, e.g. assessment and issue of appropriate Activities of Daily Living (ADL) equipment To work closely with the Speech and Language Therapist to assess and review patients with swallowing difficulties. Be aware of Health and Safety aspects of your work, and comply with any policies that may be required to improve safety within work area, including prompt reporting of accidents to Service Manager, and ensuring equipment use is safe. To undertake the measurement and evaluation of work and current practices, through evidence based practice, audit and use of outcome measures, either individually, or with team members. To comply with professional standards of practice in accordance with professional body. To maintain accurate, comprehensive and up-to-date documentation in accordance with professional standards and legal requirements, and within departmental guidelines.
