Responsibilities:
1. Assessment and Intervention:
2. Conduct thorough assessments of adults at risk, taking into account their physical, emotional, and social needs.
3. Develop and implement individualised care plans to address identified issues and risks.
4. Case Management:
5. Manage a caseload of adult clients, ensuring regular and effective communication with clients, their families, and other stakeholders.
6. Coordinate and collaborate with multi-disciplinary teams to provide holistic support.
7. Crisis Intervention:
8. Respond promptly to crises and emergencies involving vulnerable adults, implementing appropriate intervention strategies to ensure their safety.
9. Advocacy:
10. Advocate for the rights and needs of adults at risk, ensuring their voices are heard and respected in decision-making processes.
11. Collaboration and Networking:
12. Establish and maintain effective working relationships with external agencies, service providers, and community resources to enhance support services for clients.
13. Documentation:
14. Maintain accurate and up-to-date records of assessments, interventions, and case progress in accordance with organisational policies and legal requirements.
Qualifications:
15. Bachelor's or Master's degree in Social Work
16. Proven experience in adult protection and safeguarding.
17. Strong knowledge of relevant legislation and regulations.
18. Excellent communication and interpersonal skills.
19. Ability to work independently and collaboratively within a team.
20. Compassion, empathy, and a commitment to promoting the well-being of vulnerable adults.
