We are looking to recruit an enthusiastic anddynamic ANP or ACP to join our growing multi-disciplinary primary careteam. You will work within our clinical team to deliver highquality patient care.
This permanent position is offered on a full-timebasis but part-time will also be considered. Protected learning time,CPD, and support from a dedicated GP supervisor will be provided to help yougrow and enhance your skills within the role.

Manor Practice provides primary care services to a patientpopulation of 12,500 patients based across 2 sites within Wallington. ThePractice is currently experiencing rapid growth and is undergoing a transitionperiod having recently merged with another practice.

Job responsibilities
The ANP or ACP will work autonomously within theirprofessional scope of practice providing care for patients face to face, bytelephone consultation and via home visits.
The postholder will operate within their own sphere ofcompetence and agreed acceptable limits of practice to provide expertprofessional clinical advice to patients, carers and colleagues and ensuringclinical safety and excellence at all times.
The ANP or ACP will prioritise and triage the needs ofpatients and make any necessary referrals for advice, treatment orinvestigations in the appropriate manner.
The postholder will use skills, knowledge and competenciesas a fully qualified ANP or ACP and non-medical prescriber in order to beresponsible and accountable for managing patient caseloads, referrals,admission to hospital if required and discharge from care as needed.
In accordance with the practice timetable, the post-holderwill undertake a variety of duties, including:
face to face consultations, telephoneconsultations and answering queries
visiting patients at home in relation to acuteand chronic presentations
participating in triage and reviewing incomingpathology results and clinical letters
prescribe and review medication that isappropriate to the patient needs and in accordance with the latest evidencebased practice, national and practice protocols and within the scope ofqualified practice
dealing with paperwork and correspondence in atimely fashion
Assessing the health care needs of patients withundifferentiated and undiagnosed problems
Screening patients for disease risk factors andearly signs of illness
Developing care plans for health in consultationwith patients and in line with current practice disease management protocols
Providing counselling and health education andsupporting health screening programmes
Recording clear and contemporaneous consultationnotes to agreed standards
Collecting data for audit purposes
Acting appropriately in relation to alerts, QOF,medicines management, local enhanced services and other reminders during aconsultation
Undertaking special health care reviews such asSMI health checks, Learning Disability health checks and cancer care reviews.
Responding to medical problems presentedincluding history taking, examination, organising referral and forinvestigation and treatment elsewhere as appropriate
Providing clear, confident explanations topatients about their clinical management plan and ensuring that the patient isaware and in agreement with this plan
Justifying the choice of medication recommendedand understanding the impact of co-morbidities and other medications,poly-pharmacy on agent choice and prognosis
Adhering to the practice prescribing formulary,local and national guidelines
Ensuring that the use of EPS is maximised
The role may also require:

Contraception and fitting of IUDs
Routine and other adult and child immunisationsand vaccinations
Removal of sutures/clips
