Due to the expansion of the Sefton Paediatric Continence team, there is an exciting opportunity for a Band 6 Paediatric Continence nurse to join the Integrated Children's Community Nursing team (ICCNT). The post holder will have continuing responsibility for the management of a defined Child/Young Person Caseload and delivery of Specialist care for patients who have been referred to the Paediatric Continence Service across Sefton.
They will be responsible for the continuity of care.
The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload.
This will be done via a holistic assessment of care needs and the provision of clinical care for patients/clients with additional & complex needs.
Practice autonomously and demonstrate evidence-based clinical decision making.
Work within guidelines to identify and manage risk, reporting identified risks to the Continence team Lead.
Ensure the delivery of the quality strategy within the designated area incorporating the establishment of systems and processes.
Main duties of the job
1. Communicate with the multi-disciplinary team to provide best outcomes for children & young people, inclusive of safeguarding cases related to highly complex cases.
2. Provide advice to children/YP, parents/carers on a wide range of health issues to include health promotion, lifestyle advice, and health care to enable them to support the child/young person's needs.
3. Develop partnerships and joint working within other agencies as appropriate.
4. In conjunction with Senior Management, develop new ways of working to provide a responsive service to patients with both planned and unplanned care needs.
5. Support Senior Management in the implementation of practice development, improvement, and service modernisation initiatives.
6. Support the implementation of any initiatives to improve quality within the clinical governance framework.
7. Responsible for the provision of appropriate, high-quality care to a defined caseload.
8. Responsible for workload planning. Undertake a holistic assessment of need for the child/young person (YP) and their family, using the appropriate model of care and evidence-based practice.
9. Provide information to the Continence Team Lead and Community Matron on patient activity as required to promote effective service management.
10. Accurately record & maintain health records in line with NMC & Trust policy.
11. Maintain registration in line with professional bodies.

* Registered Nurse or AHP
Desirable
* oV300 Prescriber, if required for the post or willingness to undertake.
* oPaediatric Clinical Exam Module or willingness to undertake.
* oTeaching/mentorship qualification or equivalent or equivalent work-based experience.
Experience

* oPost registration experience of managing a Child based caseload.
* oUnderstanding of the principles of care management.
* oEvidence of being able to communicate complex information and advice on healthcare to patients/clients, carers, and colleagues.
* oExperience of successful multi-agency working.
* oDemonstrable experience in relevant area of practice or equivalent.
