Job summary Community Rehabilitation Physiotherapist (Pulmonaryand Cardiac Rehabilitation) Band 6- 37.5 hours per week To provide a high standard physiotherapy service to patients in the specialist area of rehabilitation. You will be expected to complete holistic assessments and support people with the management of their respiratory or cardiac condition. You will prescribe exercise rehabilitation appropriate to the patient's requirements and deliver education on condition management and healthy lifestyle choices. In addition, you will be responsible for supporting the team lead in service improvement projects and audit completion. Main duties of the job To undertake all aspects of clinical duties as an autonomous practitioner. Be professionally and legally accountable for all aspects of own workload, including the management of your patient episode of care. To be responsible for your own caseload and a defined area of service, working without direct supervision. Supervision takes the form of regular formal training and clinical reasoning sessions, peer review and case conferences. Access to advice and support from a senior physiotherapist is available if required, clinical work is not routinely evaluated. Undertake a comprehensive assessment of patients including those with diverse or complex presentations/multiple pathologies; use advanced clinical reasoning skills, to provide an accurate diagnosis of their condition.
