A bit about the role Action for Children’s Early Intervention Family Support Service works across Omagh and Fermanagh supporting children and young people aged 0-17 and their families. We have a mixed staff team of social workers and family support practitioners. Staff provide services from our Omagh and Enniskillen bases. Services are delivered on an outreach basis to communities and individual family homes, including evenings and weekends providing support when most needed. The Service is funded by the Western Health and Social Care Trust and is required to support children young people and their families at an early intervention level. We also work closely with a number of partner agencies including the Western Trust Gateway teams as well as schools, paediatrics and health visitors. Based in one of our offices in Omagh or Enniskillen you will work as part of a Team and on your own initiative to support children, young people and their families to deal with any difficulties and work together at an early stage to explore solutions and prevent long term problems. How you'll help to create brighter futures You will work as part of a Team to provide, flexible, creative, outcomes focused supports and guidance to children, young people and their families. You will undertake individual needs-assessments, develop individually tailored, outcomes focused support plans and evaluate service impact for individuals. You will provide interventions including individual direct support and evidence based group work programmes. You will support the Service Co-ordinator in promoting the service and its activities to a wide range of statutory, voluntary and community based agencies and organisations. Let's talk about you Ideally you will have: NVQ Level 3 in Health & Social care, early years, youth work or relevant subject Experience of delivering support to children and families in their own home Excellent communication skills and the ability to build a rapport with a wide range of people Access to a car with business insurance.
