Job summary We are looking for an experienced, enthusiastic, friendly and professional Practice Nurse to join our team at Pontesbury & Worthen Medical Practice on a permanent contract basis. Applicants must have some experience of working in a GP Practice, and must have completed their fundamentals training. The successful applicant would be working alongside our current nursing team, and be responsible for the delivery of practice nursing services to our patient population. The role will include managing areas such as health promotion, chronic disease management, health prevention, as well as supporting the nursing, clinical, and management teams in achieving QOF targets. Main duties of the job We are looking for someone with good primary care experience, and an enthusiastic team player who will, as well as being patient focused, will work well with the whole practice team and maintain a friendly but professional conduct at all times.
