The Fremantle Trust have an exciting opportunity for a Supported Living Team Leader to join our team on a full-time basis, based at our service in Chesham, Buckinghamshire.
Supported living is a concept that supports people to live independently who may still require help with personal care. The Fremantle Trust provides innovative and person-centred care for adults with a learning disability through a tailored support programme. Our fundamental principle is to give every single person we support the toolkit they need to live life to the full and as independently as possible. The people we support live in their own homes which may mean shared housing, individual flats and general community housing.
The main purpose of the Team Leader role is to provide leadership and management of a defined service group within a Fremantle service for people with a learning disability. Reporting directly into the Service Manager, the post holder will be required to demonstrate their passion for care by providing a high-quality standard of support to people using Fremantle services and ensuring this is delivered in line with the standards set out by the Trust, CQC, Local Authority and other relevant regulatory professional bodies.
With previous experience of the care sector and effective people management skills, the post holder will be expected to demonstrate their commitment to delivering the Trust values and aims.
Principal tasks will include:
1. Providing respectful high-quality support and care for people who use Fremantle services in accordance with their individual Support Plans.
2. Line manage the staff team ensuring effective performance management and deployment of staff for a defined group within the service and ensuring they provide excellent care for people using Fremantle services.
3. Manage the operational and administrative duties as responsible person for the whole service on a rota basis under the guidance of the manager.
4. Undertake the lead coordinating role for the whole service in specialist areas such as training, epilepsy, autism awareness.
5. As key worker for people who use supported living services, develop a person-centred support plan with their participation. Ensuring their assessed needs and wishes are comprehensively met.
6. Ensure support plans are kept up to date, obtaining appropriate input from relatives, other professionals and staff colleagues.
7. Support people to take their medication as prescribed and ensure this is recorded in the individual Medication Administration Record.
8. Ensure that people have companionship and social, emotional and leisure support in accordance with their preferences and the support plan.
9. Assist people who use services to take a full and active role in their community should they wish. Engaging with the wider community in order to create opportunities for meaningful occupation.
10. Support people who use services in working with registered social landlords to maintain their tenancies.
11. Work with specialist benefits advisors to support people who use services to access all benefits to which they are entitled.
12. Support people who use services to access comprehensive health care services.
13. Support people who use services to maintain a healthy lifestyle including diet, exercise and lifestyle choices.
14. Record essential information in daily reports, care and health monitoring charts, accident and incident reports, and service message systems clearly, accurately and immediately.
15. Implement Fremantle’s Performance Development and Support Policy. Ensure all staff receive a comprehensive induction, probationary period, performance and development reviews and participate in the in-house staff development programme.
16. Ensure that day-to-day tasks and routines are carried out to contribute to the smooth running of the service as a whole.
17. Actively promote the service and Fremantle by clear, respectful and helpful communication with all potential customers including people who use services, relatives, health and social care professionals, other officials and contractors, visitors as well as Fremantle colleagues.
18. Contribute to the success of the whole staff team by attending and contributing in team meetings.
19. Any other duties consistent with the business of Fremantle, the job, and hours of work as may reasonably be required.
