Exciting opportunities have arisen for registered nurses to join our forward-thinking Community Nursing Service within Central Cheshire Integrated Care Partnership (CCICP). There are flexible opportunities within our community nursing teams in Northwich, Sandbach, Alsager, Crewe, Nantwich, and Winsford.
Full training will be provided to support the development of community nursing clinical skills.
As part of a team, you will be responsible for the assessment of nursing care needs, implementation and evaluation of care programmes, and the delivery of high-quality holistic care within the community setting, and within our wound care clinics. You will be expected to support your peers, the team leader, and the organisation in promoting the Trust's values and behaviours.
You will need to be able to work independently, possess good time management and organisational skills, and be expected to be innovative and flexible to work in other areas if required to meet service need.

Main duties of the job
We are looking for Band 5 staff nurses to support community patient care in the Northwich, Sandbach, Alsager, Crewe, Nantwich, and Winsford Care Community Teams. The integrated team consists of Complex Case Practitioners, District Nurse/Case Managers, Macmillan, and Therapy teams delivering high quality, efficient, and safe care.
We are looking for enthusiastic, caring, and compassionate people who possess a good clinical skill set. You will also be able to holistically assess patients' needs while ensuring health promotion and quality improvement to meet the needs of our patients. You must have a good knowledge of modern IT software applications.

Job responsibilities
1. To work under the direction of the Community Nurse Team leader and be responsible for the assessment and management of the health needs of an identified population.
2. To support and maintain patients in their own preferred place of care, enabling them to maximize their independence and optimize their quality of life.
3. To liaise with all healthcare professionals and other agencies to ensure appropriate and effective service delivery.
4. To assess, provide, and review holistic nursing care, advice, and information to patients and carers within the home environment or appropriate care setting in a safe and professional manner using evidence-based knowledge.
5. To provide skilled nursing care and ability to undertake clinical tasks including wound care, leg ulcer assessment and management, administration of medication via syringe pump, care/use of intravenous lines, venepuncture, and catheter care.
6. To promote clinical excellence and evidence-based approach to care by complying with the agreed Trust policies and procedures and maintaining clinical competencies in line with the competency framework.
7. To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines.
