An old established town house, GP surgery looking to employ a new practice nurse to the team, which still gives the "family doctor" feel with face-to-face appointments.
We want to continue improving patient access, patient experience, and health outcomes and look forward to another team member joining the existing friendly, supportive nursing team to do this.

Main duties of the job
Responsible for the clinical areas of the primary care nursing team within a GP practice, delivering general treatment room services including cervical screening, child immunisations, health promotion, chronic disease management, wound management, travel services, ear irrigation/micro suction.
Ensuring that the nursing services are delivered effectively to the patient population and working with the practice multidisciplinary team in accordance with clinical-based evidence, NICE, and the NSF.
Supporting patients through their diagnosis and signposting to the relevant support agents.

Ran by three Partners, 2 are Male and 1 Female, supported by three salaried GPs, 2 are Female and 1 Male.
Currently, the nursing team consists of a very experienced ANP who has served many years within the practice and developed skills from being a Practice Nurse to becoming an ANP; an experienced Practice Nurse who has also served many years with the practice having started her nursing career as a midwife and then 2 Health Care Assistants.
The practice is managed by a full-time practice manager and there are several part-time staff who cover reception, administration, and secretarial roles.
Attached to the surgery is an MSK consultant and extended access GP and HCA.
The surgery opens at 7.30 a.m. for surgeries starting at 7.40 a.m.; on a Monday, one of the Partners offers extended hours until 7.30 p.m. but otherwise, the surgery is closed at 6.30 p.m. We are open Monday to Friday unless we are carrying out immunisation programmes.
We are keen to have staff who may like to extend their experiences and qualifications by learning new skills; we are also involved in our local PCN and their development of training practices.
Job responsibilities
1. Support the Infection Prevention Control (IPC) lead and work to the IPC Policy supporting the implementation of audits, training, support, and evidence of compliance to the practice manager.
2. Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
3. Develop, implement, and embed health promotion and wellbeing programmes.
4. Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
5. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
6. Provide routine nursing care to patients in accordance with clinical-based evidence, NICE, and the NSF.
7. Diagnose and manage acute and chronic conditions as required.
8. Prescribe medication for patients in accordance with PGDs and own scope of practice.
9. Review patient medications to enhance compliance.
10. Provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages.
11. Coordinate the provision of travel medicine services.
12. Request pathology services and process pathology results as required.
13. Oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
14. Maintain accurate clinical records in conjunction with extant legislation.
15. Ensure the use of templates by all nursing team members.
16. Ensure the appropriate maintenance and use of chronic disease registers.
17. Carry out cervical screening.
18. Develop, implement, embed, and participate in well man clinics.
19. Prioritise health issues and intervene appropriately.
20. Support the team in dealing with clinical emergencies.
21. Recognise, assess, and refer patients presenting with mental health needs.
22. Implement vaccination programmes for adults and children.
23. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).
24. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
25. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
26. Support the clinical team with all safeguarding matters, in accordance with local and national policies.
Person Specification
Qualifications
* Registered Nurse with the Nursing and Midwifery Council.
* Qualified in Phlebotomy.
* Immunisation for childhood vaccinations.
Knowledge and skills
* Ability to work under pressure.
* Ability to time manage.
* Use of GP computer systems.
* Awareness of infection control.
Experience
* Previous Nursing team experience.
* Experience in chronic disease management including diabetes, hypertension, and respiratory conditions.
* Imms/Vacs trained, including childhood immunisations.
* Experience in primary care.
* Experience of a GP practice.
* Qualified smear taker.
* Spirometry experience.
