An exciting opportunity has arisen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a Registered Nurse. The REACT team is a multi-disciplinary integrated community service within ESNEFT, incorporating OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours), and Virtual Ward pathways patients.

The role involves traveling throughout East Suffolk delivering acute care in patients' homes as part of the REACT nursing team, during core hours and out of hours. The post holder will work in an internal rotation, including early shifts, late and night shifts, weekends, and bank holidays. During out-of-hours shifts (18:00-08:00), the Nurse will be accompanied by a Healthcare Assistant. Pool cars are available for staff, and a valid driving license is essential.
Main duties of the job
* Assess, plan, and deliver healthcare for patients, including those with long-term conditions, palliative care, and rehabilitation needs, to promote quality of life and independence where possible.
* Work with the multidisciplinary team to deliver Outpatient Parenteral Antibiotic Therapy (OPAT) in the community to facilitate hospital discharge or prevent unnecessary admissions.
* Collaborate within the integrated team to prevent unnecessary hospital admissions.
* Coordinate with healthcare professionals and statutory/non-statutory agencies to provide seamless, integrated services to patients.

Qualifications
* RN Qualification (Part 1 of the register) or equivalent
* Current NMC registration
* IV administration/cannulation
* ENB 998/mentorship preparation or equivalent
* Mentorship qualification is preferred but not essential at interview
* Previous community-based experience
* Venepuncture

Salary: £29,970 to £36,483 per annum, pro-rata.
