Actively support the development of the digital platform (Joy), with a focus on developing the Directory of Services, and the successful reporting of service and patient outcomes using the management reporting capabilities of the platform. Working in Partnership with the VCSE and community to promote the Directory and onboard organisations to the platform. Develop and manage new PCN projects including introduction of new pathways and services. Drive the development of partnership and community engagement initiatives which address health inequalities. Positively promote best practice and learnings from elsewhere. Work closely with the PCN Clinical Directors, third sector partners and providers to strengthen and develop the model of Social Prescribing roles across the PCN and in practices. Establish personalised care roles and promote across the PCNs and partners. Engage with all PCN member practices and PCN Clinical Directors to ensure the integration of roles within General Practice This role will provide a lead Social Prescribing Role across all PCNs where the postholder supports PCNs/social prescribing agencies through unified approaches, guidance, supervision and support this is not a day to day management role, it is a true coordination role Establish connections and relationships with local health and care provider organisations to understand landscape, ethos, ways of working and how the team will work within a multidisciplinary team. Build an understanding of local processes, referral pathways and reporting mechanisms to understand how the role can support communication of patient progress and outcomes across systems/providers. Support future recruitment. Use materials, approaches and systems to promote Social Prescribing and Health & Well-Being Coaches within the PCN, role in personalised care, and the wider determinants of health. Supporting the Social Prescribing platform, including case management, referrals, dashboard and marketplace function. Develop effective working relationships across Primary Care Networks ensuring high visibility of the service. Liaise, represent, and promote the service to external stakeholders as necessary. As part of the PCN multi-disciplinary team, build relationships with staff in GP practices within the local PCNs, attending relevant MDT meetings, giving information and feedback on social prescribing, and promoting personalised care and shared decisions making with all the new additional roles. Support PCNs efforts to raise awareness of Social Prescribing and Health & Well-Being Coaches and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care. Work with PCN to forge strong links with VCSE organisations, community, and neighbourhood level groups, utilising their networks and building on whats already available to create a menu of community groups and assets. Develop effective and supportive working relationships with local VCSE organisations, community groups and statutory services, to make timely, appropriate and supported referrals for the person being introduced. Support efficient service running and maximising the resources available within the team and wider system. Team management and support Design induction and coordinate training programmes for Social Prescribing, Health & Well-Being Coaches, and other roles to ensure consistent standards of effective practice across the service. Liaison with team to ensure all staff and volunteers have access to relevant training where necessary. Set and agree standards of performance across the team ensuring staff are supported in achieving these through regular supervision, appraisals, and training. Provide 1 to 1 line management support and supervision to the team as required to support adherence to service quality assurance framework and practice. Coordinate regular team meetings to develop and maintain a mutually supportive team-working approach. Ensure that the service staff are supported in working from physical environments that are appropriate to the needs of the service and are compliant with relevant policy and regulation, and in particular encouraging good practice in lone working and health and safety. Ensure there are effective communications and team development opportunities to support service staff in operating as a virtual team across several locations and organisations. Social prescribing Act as a social prescribing link worker for one of the PCNs, embedded within a practice team(s) as appropriate
