Complex Case Manager Substantive post 37.5 hrs per week within the ICB.
Project a positive image of the CHC Team both within and outside of the service.
Responsible for undertaking CHC assessments and the management of Complex patients with identified healthcare needs.
Responsible for the line management of B6 Clinical Nurse Assessors.
Responsible for ensuring KPI's and QIPP savings.
Quality Assurance.
Oversight of Complex LD Cases.
Liaising and active engagement with CHC stakeholders.
Facilitating Level 1 dispute Meeting.
To participate with the allocated duty role within the CHC Team.
Ensuring ICB Values are achieved.
Main Duties of the Job
Complex Case Manager:
1. To assess, case manage, monitor, and formally review the health needs of those patients funded by CWICB within Coventry, Rugby, and Warwickshire North places as well as those cases who are placed out of county placements receiving care under CHC.
2. To review existing historical joint funding care packages for learning disabilities clients where the issues are considered to be standard rather than complex.
3. To carry out verification and Quality Assurance of all DSTs and Review Assessments as one of the primary assignments for the role.
4. To coordinate and provide all needed support to the Legal Team in all Legal Cases and report to the line Manager of the progress of all cases.
5. To liaise with the ICB DOLs Team in order to provide the team with any required clinical information as may be required and provide update to the Clinical Commissioning Manager of the progress.
6. To co-ordinate and ensure timely and effective dispute process. To Lead in Level 1 dispute and if not resolved, this will be escalated to Level 2 stage.
About Us
Operational Management:
1. To manage Clinical Assessors to ensure the CHC team works effectively and maximises individual performance in respect of patient care.
2. To be part of the recruitment process for members of the CHC Team.
3. Provide peer reviews for clinical staff.
4. Have access to and receive all the ICB mandatory training and have a current personal development plan.
5. Participate in and take responsibility for ensuring that those staff under your management are aware of and fulfil their responsibilities under all appropriate legislation and guidance such as the Mental Capacity Act, The Human Rights Act and Safeguarding Adults and Children Legislation.
6. Ensure that clinical staff are aware of any updates to relevant guidance (CHC) and protocols in line with DoH policy, case law and clinical research relevant to the case load and clinical discipline.
7. Undertake appropriate training and ensure that all clinical staff receive training appropriate to their role within the CHC Team.
8. To provide professional support, expertise, and input to the CHC Team and verification Panels.
Job Responsibilities
1. To ensure that all clinical staff within the CHC team are aware of and adhere to all the ICB policies and procedures.
2. Ensure that you receive the appropriate supervision and annual appraisal enabling your professional development to be maintained.
3. To assess, case manage, monitor, and formally review the health needs of those patients funded by CWICB within Coventry, Rugby, and Warwickshire North places as well as those cases who are placed out of county placements receiving care under CHC.
4. To provide specialist training, support, and education to other Health Professionals.
5. To case manage your own clinical caseload (end to end) including, but not limited to, resolving issues with funding, placements, invoices, and complaints.
6. To attend / chair Best Interest meetings and Clinical Treatment Reviews as required.
7. To coordinate and provide all needed support to the Legal Team in all Legal Cases and report to the line Manager of the progress of all cases.
8. Oversee and ensure that CHC reviews are carried out as per the National guidance and take responsibility for managing and reviewing complex cases either individually or with the CHC Nurse Assessors.
9. Demonstrate evidence-based practice.
10. To participate, inform and support continued service developments.
11. Having oversight for all the Band 6 Clinical Case Management.
12. To provide case management for individuals in receipt of 100% NHS funding to include residential, supported Living, domiciliary and respite care packages where appropriate, offer advocacy, support to the voluntary sector, and provide advice and information to both the individual and others.
13. To ensure all appropriate correspondence to families and correspondence and queries from families are dealt with efficiently.
14. To be a constant point of contact and support to those involved and lead in the case management of individual cases.
15. To deal and manage complaints and queries to their conclusion relating to your case load.
16. To provide accurate assessment(s) of need when required to support identification of appropriate services. This includes avoiding hospital admissions, increase/decrease in care and short term 1-2-1.
17. Overall, demonstrate a keen desire to improve and drive performance by making positive impact to care delivery and to CWICB.

Evidence and Verbal
* Evidence of Leadership skills.
* Clinical Qualification - RLDN.
* Previous experience demonstrates experience of leading a team or others working in multidisciplinary teams and partnership working with other statutory and voluntary agencies.
* Knowledge of working within CHC or understanding of NHS CHC Framework.
* Evidence of Leadership training or experience in a Leadership role.
