An exciting opportunity has arisen to join our multi-disciplinary PCN team. We are a dynamic, forward-thinking PCN team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.
The PCN has been successful in becoming part of a national pilot scheme which will look at ways to make General Practice Sustainable. The PCN is mobilising a Home Visiting team to support the practices with home visits. We are therefore looking to expand our team by employing Advanced Clinical Practitioners to work alongside our practices and PCN staff.
Our PCN staff currently includes Clinical Pharmacists, Physicians Associates, Social Prescribers, Paramedics, Physiotherapists, Care Coordinators, Nurse Associates, and Pharmacist Technicians.
Main duties of the job
The Advanced Clinical Practitioner (ACP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis, and treatment of patients.
The post holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills, and behaviours appropriate to the healthcare professionals (HCP) setting and job role.
The role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment within their own homes (where appropriate).
An ACP is autonomous in making decisions based on assessment, diagnosis, and interpretation of test results and will initiate, evaluate, and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice, and care.
The post holder will need to be capable of independent travel across the Rother Valley South area to provide home visits and work flexibly to meet the needs of the Business.
About us
Rother Valley South PCN is made up of four GP Practices with a weighted list size of 57,000. Based in South Yorkshire ICB, the Network covers a large suburban area of Rotherham.
The PCN formed in 2019 with no changes to its original members to date. The PCN incorporated as an LTD Company in 2023 to further support development within the network.

Job responsibilities
Main Duties and Responsibilities:
1. Conduct home visits to undertake consultations with patients.
2. Practise in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions, and omissions at this level of practice.
3. Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty, and incomplete information.
4. Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change.
5. Work in partnership with individuals, families, and carers, using a range of assessment methods as appropriate.
6. Demonstrate effective communication skills, supporting people in making decisions, planning care, or seeking to make a positive change.
7. Use expertise and decision-making skills to inform clinical reasoning.
8. Initiate, evaluate, and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice, and care.
9. Exercise professional judgement to manage risk.
10. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
11. Implement and evaluate individual specialised treatment plans for chronic disease patients.
12. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients' health.
13. Be a professional role model for the organisation.
14. Actively engage in peer review to inform own and others' practice, formulating and implementing strategies to act on learning and make improvements.
15. Actively seek feedback and involvement from individuals, families, carers, communities, and colleagues in the co-production of service improvements.
16. Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice (clinical practice, leadership and management, education, and research).
17. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
18. Maintain accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively.
19. Develop, implement, and embed an effective call/recall system.
20. Critically appraise and synthesise the outcome of relevant research, evaluation, and audit, using the results to underpin own practice and to inform that of others.
21. Develop and implement robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance.
22. Chaperone patients where necessary.
23. Support the team in dealing with clinical emergencies.
24. Support the team with all safeguarding matters in accordance with local and national policies.
25. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
26. Deliver opportunistic health promotion where appropriate.
Person Specification
Experience
* Experience of practice within the four pillars.
* Job plan that demonstrates advanced clinical practice and has equity with peers working at this level.
* Experience of prescribing and undertaking medication reviews.
* Experience of working in a primary care environment.
Qualifications
* Educated to Masters degree as detailed within the HEE document titled Multi-professional framework for advanced clinical practice in England.
* Registered on their appropriate professional register.
* Teaching qualification.
* ALS and PALS.
* ACP Digital Badge.
* Qualified independent prescriber - V300.
