Modality Partnership Mid Sussex Division are seeking a skilled and compassionate, experienced General Practice Nurse to join our healthcare team. The role can be between 25-37.5 hours per week, however, a minimum of 25 hours per week would be required. Main duties of the job
The role involves the delivery of evidence-based practice for patients presenting with chronic conditions and the provision of preventative health care to the practice population. The postholder will demonstrate critical thinking and skills in clinical decision-making in the management of patients.
This role is not a regular 9am-5pm job; it requires innovation, flexibility, and commitment. The postholder will be required to work resourcefully as part of the team to ensure tasks are completed.
Job Description
Key Duties & Responsibilities:
* To promote and maintain high standards of nursing care in accordance with the NMC Code of Professional Conduct and Guidelines for Professional Practice
* To provide travel immunisations, cervical cytology, ear syringing, wound management
* To plan and run health promotion and long-term condition-related clinics, ensuring that the care given meets local and national guidelines, policies, and standards
* To communicate directly with patients regarding their ongoing and complex health needs, demonstrating skills of tact, diplomacy, and empathy
* To ensure detailed assessments of patients' healthcare needs and that individual programmes of care are planned, implemented, and regularly reviewed
* Facilitate patient referrals to appropriate clinical services
* To liaise with other disciplines and outside agencies to provide holistic healthcare while maintaining patient confidentiality
* To participate in the delivery and evaluation of healthcare services at the Practice through meetings and regular liaison with colleagues
* To be responsible for the ordering, safe systems, and processes, storing and administering of all medicines/drugs according to NMC standards
* To undertake and participate in clinical audit projects as necessary
* Respond to medical emergencies
* Ensure adherence to the principles of Infection Control
* To maintain awareness of issues concerning child protection & vulnerable adults
Principal Activities – Education and Development
* To keep abreast of advances in clinical practice and identify personal training and development needs
* Support colleagues in their development through encouragement and informal teaching
* To attend meetings as directed by the Practice Manager and Partners
* To attend mandatory training identified through the appraisal system
Principal Activities – Professional
* Contribute and work within the clinical governance agenda
* Work within evidence-based nursing protocols
* Accurately record all healthcare activity within patients' clinical records ensuring compliance with NMC standards
* Ensure effective communication between all stakeholders, giving attention to the complex and sensitive nature of such information
* To work flexibly to meet the needs of the service as directed by senior staff and GPs
Principal Activities – Administrative
* Follow practice and NHSE/CCG policies, procedures, and guidelines
* Complete reports and forms required by the practice and NHSE/CCG
* Participate in staff surveys and contribute to research-based practice
* Be aware of statutory national requirements relevant to the job function (e.g., Health and Safety at Work Act)

Person Specification:
Qualifications and Experience:
* Minimum 1 year General Practice Experience
* Minimum 2 years post-registration experience
* At least 1 year recent community nursing experience
* Experience of nurse-led management of chronic conditions
* Experience of implementing protocols and clinical guidelines
* Experience of quality initiatives such as clinical benchmarking
* Audit experience
* Clinical supervision training and experience
Knowledge:
* Management of patients with chronic conditions
