Due to a successful Hospice UK grant application, St Leonard's Hospice, in collaboration with Nimbus Care, are recruiting a Palliative Care/Frailty Coordinator. The post will be offered on a fixed term 12-month contract.
Employed by St Leonard's Hospice and based within The Frailty Hub at Acomb Garth, York, this new and innovative post aims to ensure joined up education and care for individuals living with frailty who may be reaching the end of life.
We are looking for a Registered Nurse who has experience of working within the palliative care setting and is passionate about the delivery of high-quality end of life care for individuals living with frailty. As the Palliative Care/Frailty Coordinator, you will:
1. Assess, plan and evaluate nursing care.
2. Maintain accurate and legible documentation.
3. Communicate necessary information to all relevant team members, both internally and externally to the Hospice and The Frailty Hub and its system partners.
This is a new and exciting role and will offer the opportunity for you to take an active role in the development of this service.
A comprehensive induction programme will be provided along with clear objectives and the relevant support to achieve the outcomes of the project.
