Job Title: Resettlement Officer (Supported Housing Move-on) Department: Housing Division: Environment & Community Section: Housing Options Job Purpose & Objectives Maximise the effectiveness of this short-term post by supporting providers and clients to facilitate the transition of as many individuals as possible from supported housing to independent living. Ensure provider staff gain comprehensive knowledge of move-on options across the area to support clients effectively in the future. Promote the importance of daily activities and structure to clients transitioning into independent accommodation. Strengthen relationships between supported housing providers, housing providers, partners, landlords, and agents to secure a consistent supply of high-quality accommodation. Ensure moves from supported housing are sustainable and tailored to meet client needs. Main Duties & Responsibilities Assist people moving on from supported housing to secure appropriate settled accommodation. Liaise with housing providers, partners, and landlords/agents to secure a steady supply of good quality accommodation for those in supported placements. Upskill provider staff regarding the private rented market, landlord relationships, housing registers, and council incentives for move-on. Promote and support ongoing initiatives to attract and retain landlords/agents. Support provider staff in facilitating clients' transition to independent accommodation. Ensure transition and floating support is available for people moving on. Raise awareness of services available to support clients and provider staff for move-on and independent living. Work with provider staff and clients to highlight the importance of daily structure and activities for individuals transitioning to independent living. Ensure move-on accommodation is sustainable and meets clients' long-term needs. Support providers with practical aspects of move-on, including tenancy sign-ups, benefit applications, utility setups, furniture, and removals. Provide housing advice and assistance in line with legislation and council policies to provider staff and clients. Collaborate with supported housing providers to identify clients ready for move-on and support their timely transition. Maintain accurate case notes and record activity in Locata. Assist provider staff in developing move-out packs containing useful information for independent living. Address safeguarding and risk management plans related to move-on, adhering to council policy. Supervisory/Managerial Responsibility Communication/Contacts Service provider staff: Provide advice, assistance, and training on move-on options, services, and identifying clients ready for independent living. Clients in supported housing: Assist clients in sourcing accommodation and making arrangements for successful transitions. Landlords/Letting Agents: Secure affordable private rented accommodation for applicants. Partner agencies: Ensure partner agencies contribute to the move-on plans for each client. Revenue & Benefits, Rent Deposit Team, and DWP: Support provider staff and clients in securing financial arrangements for the move from supported to independent housing. Additional Information Examples of Creativity and Innovation: Sourcing accommodation for individuals with no tenancy history, references, or low budgets. Managing client expectations while encouraging them to pursue independent accommodation. Enhancing provider staff's knowledge and expertise in facilitating transitions from supported housing. Collaborating with provider staff and clients to develop daily structures and activities for a successful transition to independent living.
