Working in an integrated way within primary care you will support the development of nutritional knowledge and behaviour change skills of health care support workers and dietetic assistants to deliver brief intervention for those at risk of type 2 diabetes. The role is fully supported by the national programme team with programme protocols, resources, training courses for support workers and evaluation tools.
Three part time posts (18.75 hours per week) based in each of the West, Centre, and East integrated health communities are available. The option of the role on a full-time basis (i.e. across two IHCs) will be considered and may be offered if an applicant successful at interview holds an interest in this.
Main duties of the job
Clinically lead the development, implementation, delivery and evaluation of best practice in relation to diabetes prevention for the designated IHC working as an integral member of and in partnership with primary care, the national programme team and diabetes prevention leads across other IHCs in BCU.
Support others in the delivery of highly specialist seamless care to individuals and groups who are at high risk of developing diabetes; supporting management of complex cases and promoting empowerment of individuals to self-manage their condition.
Team leads delegated staff ensuring consistent evidence-based standards of care and effective multi-disciplinary working across BCUHB.
Work with the other team leads within the area to ensure that there is consistent leadership of staff - particularly in times of leave or sickness. Forge and develop relationships within the other areas of the health board.
