The IST forms part of the wider South Staffordshire offer to supporting individuals with a Learning Disability and Autism who are experiencing difficulties which may otherwise lead to placement breakdown or hospital admission. The team is reliant upon effective working relationships between services within and outside of MPFT.

Main duties of the job
You will be applying for a Clinical Nurse Practitioner role. Working as part of an established Multi Disciplinary Team, providing a valued clinical leadership role to design and deliver short term specialist interventions within the framework of an IST shared caseload.
This will involve utilising your core specialist nursing skills, whilst implementing specialist behavioural assessment to inform Positive Behavioural Support.
As an IST nurse, you will undertake a Lead Practitioner role to co-ordinate assessment and interventions throughout the IST clinical pathway with your MDT colleagues, and the service user, parent, and/or care provider, before discharging back to the referring clinician within the Community Learning Disability Team (CLDT).
We are looking for an enthusiastic and dedicated RNLD or suitably experienced nurse, with experience of delivering a professional person-centred service with the aim of achieving positive life outcomes for adults with a learning disability. The IST is part of the Specialist Learning Disability Services and receives internal referrals from CLDT colleagues. The post requires interface with a number of teams/services to promote shared formulation, including assessment and management of risks which place the individual and others at risk of serious harm or for whom the nature/degree of risk might otherwise lead to exclusion, placement breakdown, and admission to inpatient services.

You will work closely with your IST MDT colleagues to support a defined, shared caseload of adults who may be at risk of placement breakdown, including individuals in transition from children's services, specialist inpatient services or moving between community placements. You will be using flexible, creative, multidisciplinary approaches and make a significant contribution to the continued development of high quality local services and capable environments in South Staffordshire for people who have a learning disability.
Your role will include providing person centred assessment, to advise care givers, and junior staff, to implement and monitor clinical interventions based on MDT clinical formulation. On occasion this may involve planned out of hours work, with access to on-call support and supervision, if the needs of an individual require it. You will work collaboratively alongside the Specialist Community Learning Disability Teams, in-patient services, and statutory / independent sector provider services.
You can expect specialist clinical training and updates, and regular supervision and professional leadership.
As the post involves travelling across South Staffordshire, having a full UK Driving licence and car will be helpful.
