An exciting opportunity has arisen to join South Tees Hospitals NHS Foundation Trust.
We are looking for enthusiastic individuals with good interpersonal skills and a passion to deliver a high standard of care to patients affected by cancer.
You will be working alongside established teams of clinicians, specialist nurses, allied health professionals, and MDT and cancer care co-ordinators to provide care for patients.
You will be expected to work at both of our acute hospital sites in Middlesbrough and Northallerton as required and be adaptable & flexible in your approach.
Main duties of the job
The role will involve co-ordinating care from referral to treatment. You will have regular contact with patients, families, and carers to support and signpost to ensure effective progress along care pathways, in liaison with other colleagues and departments in the hospital and the community.
The role will involve supporting high quality personalised care including:
* Carrying out holistic needs assessments and care plans
* Preparing treatment summaries
* Offering health and wellbeing advice and support
* Follow up for patients following completion of treatment
You will be expected to attend the multi-disciplinary team meetings with the wider multi-disciplinary teams to transcribe MDT outcomes live/complete the MDT outcomes using NHS IT recording systems.

Person Specification
Knowledge & Skills
* NHS experience or equivalent
* Experience working with a multi-professional team including medical or clinical staff
* Experience of direct patient contact including telephone and face to face contact
* Experience in administration / organisational skills
* Previous experience of dealing with customers, patients and/or members of the public
* Recent experience working with cancer patients or in cancer services
Experience
* Excellent communication skills (written, verbal and listening) with proven experience of dealing with a wide and complex range of stakeholders and matters
* Experience of roles using IT systems, particularly Microsoft Word, Excel and PowerPoint
* Be able to interpret and follow written instructions from members of the multi-disciplinary team
* A basic knowledge of cancer and cancer treatments
Qualifications and Training
* GCSE in Maths and English (A-C/4-9) or Functional Skills (level 2)
* Knowledge of secretarial and administrative procedures and systems, the majority of which are non-routine, acquired through training and experience to Vocational Level 3 or equivalent; plus knowledge of specialist functional/medical/legal terms, organisational policies and procedures and knowledge and experience to deal with non-routine issues such as problem solving for an area of work or developing alternative or additional procedures.
