The post holder will: Work within community services and support the multi-disciplinary team (MDT) for Neighbourhood Rehabilitation Service. Contribute in ensuring that safe care is provided to patients within a community setting. Enhance the admission and discharge processes for patients and their families/carers. Support the clinical team with the admission to and discharge from Neighbourhood Rehabilitation Service; ensuring a smooth transition throughout the appropriate elements of community Intermediate Care services. Work in close liaison with the Intermediate Care patients key clinician / MDT in ensuring an efficient admission process and effective discharge planning and care. Assist in the provision of administrative support to the service. To work with the service, its staff and wider services to help facilitate the safe and timely admission and discharge of patients. To support the therapy and nursing team to carry out initial and follow up interviews with patients and carers in order to collect accurate information regarding home circumstances and pre-admission abilities. To provide the patient and their family/carer with information regarding their estimated discharge date, co-ordinated alongside the therapist / nursing team responsible for their care plan. To have responsibility for keeping accurate and up to date records relating to patient care plans and discharge plans; keeping all relevant personnel (including RightCare Barnsley) involved up to date with any potential issues or delays. To develop a rapport with patients and their families/carers, and encourage their involvement in the progress and planning towards discharge. To assist in arranging and finalising any transfer or discharge arrangements including transport for the patient, if required. To assist in arranging and ordering equipment for patients following assessment by clinicians. To communicate effectively with patients and their families/carers, acknowledging the individual circumstances and potential sensitivity of the situation, adjusting communication style as appropriate. To support both qualified and non-qualified nursing and therapy staff in the safe management of patient care. To act as a link person between the patient, their family/carer and all other members of the MDT involved with the patients care plan and discharge preparation. To communicate immediately any potential problems regarding a patients discharge to the appropriate person to ensure that problems are escalated to a senior clinician and the process is proactive. To participate in and contribute to MDT meetings and discussions; as a member of the Neighbourhood Rehabilitation Service. To assist in covering the duties of other Administrative Officers in the team as required. To assist with the administration and day to day running of the service, utilising appropriate clinical and IT systems, including electronic records. This will also include review of information to identify and correct any data quality issues. To participate in appropriate meetings and staff development as appropriate. To maintain professional behaviour and appearance, appropriate to the service area.
