The Clinical Nurse Specialist (Hospital Discharge Team/Community UCR) works as part of an integrated team alongside our DCC Social Care/DPT teams and other health professionals to ensure patients are discharged from Hospital either back home or to another setting promptly, effectively and safely. The team is the central point for referrals and is responsible for the initial triage of patients for pathways 1, 2, and 3, and they are also a key link with families and other health professionals.
This post plays a key role in facilitating a safe discharge from Hospital for our patients and is essential in maintaining patient flow.

The Hospital Discharge team has an exciting opportunity for enthusiastic and motivated registered nurses. If you would like to come and join a supportive and dynamic multidisciplinary team, then look no further.
The Hospital Discharge Team is an integrated health and social care team based on the acute site providing clinical assessment and triage for patients face to face to prevent unnecessary hospital admissions or to facilitate appropriate discharges. We also work to support patients through the end stages of their life. This role involves using a patient-centric approach to completing nursing assessments and formulating discharge plans.
This post is being advertised as rotational with the intention of rotating into Intermediate Care in the community. As such you will need to be a car driver to apply. This position could be in any of the 6 localities across eastern Devon.
The successful candidate will have the ability to work as a confident autonomous practitioner. You will need high-level clinical assessment and problem-solving skills to manage complex patients who require assessment of needs prior to discharge. This includes face-to-face assessments of patients on the wards and linking with the MDT, through attendance at board rounds and other resources.
This post allows you to draw on skills gained from both acute and community experience, due to the broad range of patient presentations and work both over the phone and face to face in the community and acute hospitals.
You will also work with Specialist Nurses and Physiotherapists, enabling you to expand your knowledge in multidisciplinary complex care. You will need to demonstrate flexibility, good time management skills, and excellent communication and interpersonal skills.

Working Pattern:
The hours needed for the position are 37.5 hours per week between the times of 8 am - 6 pm. You will form part of a rolling staffing rota, covering the service 7 days a week, and flexibility in your working is appreciated. You will be required to work one weekend in every four to support the team.
