Job summary
We are looking for anenthusiastic and motivated Band 7 Clinical or Band 8a Advanced Clinical Pharmacist(depending on experience) to be part of the Kirklees primary care mental healthpharmacy team covering the maternityleave of the existing post holder. This post is a fixed term for 12 months.
The role involves workingalong side advanced clinical pharmacists and medicines optimisation techniciansto provide specialist mental health clinical pharmacy services for GP practicesand service users to improve access to information on mental health medicinesand support appropriate prescribing and safe use of medicines.
The postholder will work withKirklees Primary Care Networks liaising with both primary care and secondarycare colleagues to support those with mental health conditions being managed inprimary care.
The post is also part of thewider South West Yorkshire Partnership Foundation trust pharmacy team withaccess to support, training and supervision from experienced mental healthpharmacy staff.
Main duties of the job
Therole of the post holder will include provision of direct patient care, carryingout patient consultations and medication reviews for mental health medicines,rationalising and optimising medicines for mental health conditions, providingsupport regarding mental health medicines for those with long term physicalhealth conditions, prescribing where appropriate and qualified.
Therewill also be a role to support other health care professionals in relation tomental health medicines including responding to queries and education andtraining.
The post holder will workclosely with other members of the team supporting medicines reconciliation ondischarge from mental health services, shared care management andoverprescribing medication reviews.

To participate in the provision of a safe and effective specialistclinical pharmacy service to designated wards within SWYPFT.
To assist Principal Pharmacist or Senior Clinical Pharmacist in theoverall provision of pharmacy services to units or teams within SWYPFT.
To deputise for PrincipalPharmacist or Senior Clinical Pharmacist as appropriate

The postholder will, as an Advanced Clincial Pharmacist(ACP), contribute to the strategic direction forthe development of clinical practice within specialist mentalhealth pharmacy primary care andacross the Trust as required.
Leading on services consolidation aswell as expansion in line with service and trust objectives and in partnershipwith the Clinical lead and General manager .
The post holder will take the lead in extending boundaries and creatingframeworks for the enhancement of clinical practice within their speciality.
The post holder will continue to strive towards excellence in clincialcare and communicate this excellence to colleagues both within the organisationand externally. Work as an independent non-medicalprescriber within areas as required and designated, taking responsibility forthe clinical assessment of the patient, establishing a diagnosis and theclinical management required in accordance with the Trusts non-medicalprescribing policy and protocols.
