- To povide specialised physiotherapy assessment, education, advice and treatment to adult patients in community referred via Hillingdon stroke early supported discharge pathway without direct supervision. To work in a variety of community locations including patients homes, nursing homes, and residential homes and provide intensive rehabilitation To educate and advise patients, their families and carers in order to facilitate self-management of their condition and optimise their rehabilitation potential and quality of life. To independently manage a defined caseload of complex cases contributing to the provision of a rehabilitation service To support in the development of new initiatives within Central and North West London NHS Trust and London Borough of Hillingdon Adult Social Care. To be an active member of the multidisciplinary team working collaboratively with other members of the team and recognising the skills and expertise of others. To deputise for the band 7 stroke ESD clinicians for smooth running of stroke ESD team. To undertake seven day working To be proactive in personal professional development. To provide support and supervision to Band 5 physiotherapist, rehabilitation support workers, therapy technicians and delegated under graduate physiotherapy students, including monthly supervision and annual appraisal. To accept clinical responsibility for a designated caseload of patients and to organise this effectively and efficiently with regard to clinical priorities and use of time.
