Main duties of the job
Main Responsibilities:
1. Lead the daily operations of a team of community nurses and HCAs.
2. Ensure safe and effective allocation of work across all teams.
3. Line manage a defined group of colleagues in the integrated team .
4. Oversee the budget, including processing invoices and expenses.
5. Ensure safe staffing levels and maintain the E-roster system.
6. Facilitate regular team meetings and participate in Care Home MDT meetings.
7. Respond to incidents and address any safeguarding concerns.
8. Provide caseload supervision.
9. Collaborate regularly with other healthcare professionals, including GPs, physiotherapists, occupational therapists, social workers, and care providers.
10. Monitor and support staff wellbeing.
11. Provide clinical support to the team and student nurses when needed.
12. Ensure staff maintain competencies and receive ongoing education and training.
13. To work closely and collaboratively with the Care home service leads for ongoing innovation and service development.
14. Develop relationships between the Care Home service and the Care Home Staff to drive up quality and working relationships to improve confidence across the Care Homes and the Care home service.

7. Dimensions and Context of Role
The District Nurse Team Manager is responsible for the operational management of the locality-based District Nurse Team.
The role entails 24/7 responsibility, 365 days a year, for the District Nursing Team, supported by Band 6 nurses.
The manager provides leadership in service development and redesign, profiling local health needs within the locality, and identifying or addressing resource needs.
Active involvement in the strategic planning of health and community-based services, with responsibilities including deputizing for and supporting the Operational Lead / Directorate Manager and Professional Leads.
The role includes providing clinical leadership and fulfilling clinical duties within the team.
9. Key Tasks and Responsibilities Clinical Responsibilities:
9.1Ensure the privacy, dignity, and human rights of all patients and carers are respected.
Support the team in managing complex or urgent situations, such as NHS continuing healthcare and safeguarding concerns, in collaboration with patients and carers.
Assist caseload holders in managing and coordinating care packages.
Oversee the quality assurance of continuing NHS healthcare assessments within the team and across the locality.
Identify and challenge professional and organizational boundaries, developing and implementing improvements to ensure high-quality, patient-centred care.
Work with integrated multidisciplinary teams across health and social care to develop, implement, review, and evaluate care programs for patients and their carers.
Ensure care provided and services delivered align with national, local, and organisational targets.
Engage in health promotion activities for all age groups and contribute to planned health promotion initiatives.
Maintain accurate, confidential, up-to-date, and legible records of care provided, adhering to organisational policies, NMC guidelines, and government directives.
Oversee the quality of patient records through audits or peer reviews, and hold regular discussions with team members.

Serve as the operational manager for a locality-based District Nursing Team.
Set workload priorities for yourself and the team, balancing patient care, management responsibilities, and staff training needs within resource limitations.
Report, record, and manage all incidents and near misses related to health, safety, security, fire, physical violence, aggression, and verbal abuse.
Maintain a visible and accessible presence for both patients and staff within the organization.

Oversee duty rotas, ensuring effective use of resources, adherence to the roster policy, and rigorous control of bank and agency usage. Manage staff sickness and other leave while maintaining safe staffing levels.

Ensure that staff complete and submit all necessary forms, documentation, and IT data in a timely manner as required by the manager/organisation.
