Job summary The practice is friendly, forward thinking, operating Emis Web clinical system and is committed to providing a high quality service to our patients. You will work closely with our Practice Pharmacist, GPs, other practice staff and members of the primary health care team. You will support, promote and implement high quality, evidence-based, cost-effective prescribing. This includes patient medication reviews, helping the practice achieve prescribing targets and supporting the implementation of national and local guidance in order to improve patient care. Main duties of the job The role will be essential to supporting prescribing excellence, medicines optimisation and helping to resolve prescribing queries. With responsibility for patient care, you will conduct medication reviews to ensure the most appropriate and effective use of medicines. And, in consultation with both patients and doctors, you will agree action plans to enable the seamless transfer of care.
