We are looking for Care Navigator(s) to join our experienced team. To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practice's multidisciplinary team. The care navigator will also be expected to undertake reception duties as part of the role, supporting the administrative team in delivering a polite and professional service to the entitled patient population.
We are a training practice for trainee Doctors and medical students, with a supportive team.
Our Patient Participation Group (PPG) is very active and supportive in the running of the practice.
Main duties of the job
The following are the core responsibilities of the Care Navigator:
1. Process and signpost patients
2. Initiate contact with and respond to requests from patients, team members, and external agencies
3. Enter read-code data on Emis
4. Manage registrations
5. Input data into the patients' healthcare records
6. Direct requests for information to the appropriate team
7. Manage queries as necessary in an efficient and polite manner
8. Carry out system searches as requested
9. Maintain a clean, tidy, effective working area
10. Monitor and maintain the reception area and noticeboards
11. Support clinical staff with general tasks as needed
12. Participate in practice audit as directed
13. Action incoming emails
14. Complete opening and closing procedures in accordance with the duty rota
15. Support pharmacist(s) in the management of prescriptions
16. Manage the movement of patient medical records
17. Attend small group and team meetings
18. Monitor and replenish supplies
19. Medical Record Envelope management
20. Support the surgery team to aid the smooth running of the business

Qualifications
