Job summary Harrow Network Partners Harrow Network Partners Ltd (HNP) is a new entity created to represent the 5 Primary Care Networks (PCN) established in Harrow. Collectively, the PCNs represent the 32 General Practices operating in Harrow and providing valuable care to over 260,000 patients. HNP was recently successful in securing two contracts which will run concurrently over the next 2 years. The post holder will take a lead role in the development of Harrow Network Partners Ltd. They will ensure the delivery on a wide range of business, transformation and improvement initiatives across the company providing strategic support and leadership particularly focussing on the Harrow Training Hub and the CRM Hub pilot. The post holder will develop strong relationships and engage with a wide range of stakeholders across Primary Care in Harrow to ensure collaborative approaches to pan Harrow development, service development and delivery. The post holder will be responsible for managing agreed projects with a focus upon developing the partnerships across Primary Care and Harrow PCNs, community services, social care, mental health, the voluntary sector and secondary care. A successful cardiac renal metabolic hub will aim to prevent onset, progression and deterioration of long term conditions particularly CKD, working towards reducing the long term demand on dialysis and transplantation, with a primary focus on preventing progression of early disease. The initial phase of delivery to general practices will begin in Oct/Nov 2024 for the first phase of the pilot. The post holder will be to provide effective project management support to the Clinical Lead and guidance to the participating practices. The effectiveness will be evaluated with the view to developing a sustainable model that is scalable across NW London.
