We are looking to appoint a dynamic, motivated & enthusiastic Therapy Liaison to work within our Neighbourhood Rehabilitation Service.
You will work as part of a multi-disciplinary team of Allied Health Professionals who work across the neighbourhood rehabilitation pathway in the community.
The Neighbourhood Rehabilitation Service provides short term intensive rehabilitation following an injury or illness. This may be following discharge from hospital or to prevent a hospital admission.
As Therapy Liaison you will be responsible for enhancing the patients pathway through the service and facilitating a smooth discharge process for patients and their families/carers.
You will support the clinical team with patients who have a range of requirements, this could include the co-ordination of social care support, finding access to local agencies which offer ongoing support and wrap around support from core health services., As Therapy Liaison you will, The team is based at Kendray Hospital however you will work across the Barnsley area therefore being a car driver & having access to a car is essential to the role.
The role is 37.5 hours, Monday to Friday up to 8pm; however, you may be expected to work some weekends. This may vary due to the needs of the service.

Have excellent communication & negotiation skills.
Liaise with patients, family & members of the multi-disciplinary team on a regular basis, including care home staff, therapists, nurses, GPs & Advanced Nurse Practitioners.
Have a varied workload.
Have a keen interest in speaking with patients & improving their experience within Intermediate Care.
Be competent with IT systems; experience of NHS electronic patient record systems would be advantageous.
Demonstrate an ability to work in a busy & often pressurised environment.
