The unit provides a Comprehensive Geriatric Assessment to the frail elderly patients living within the Buckinghamshire area, additionally we have an Intravenous Infusion Lounge.
The post involves working within a Multidisciplinary Team to provide nursing interventions to frail elderly patients providing care to avoid hospital admission as well as facilitating discharges from the acute settings.
You must be innovative in your approach and willing to work with other members of the Multidisciplinary team and to ensure a high-quality service delivery. You must be willing to develop your own knowledge and skills, you will be supported in your personal development with training.
Experienced and newly qualified staff nurses will be considered for the post. Working days are Monday-Friday, no weekends, nights, or bank holidays.
Acts always in accordance with the NMC Code of Conduct, and within the principles of the scope of Professional Practice and ensures other members of the team also work within these.
Maintain professional working relationships with all members of the Multidisciplinary Team and effectively contribute to the team’s continual development.
Undertakes the nursing assessment of the patient including risk assessments e.g. Moving and Handling and nutrition
Undertake drug administration and medicine management.
Ensures planned nursing care is delivered safely and effectively taking into account the views of the patient and their relatives. Advise on health promotion and the prevention of illness.
Maintains absolute confidentiality in respect of all medical and nursing records and also interpersonal issues with colleagues.
Interpret the significance of the patient’s clinical observations and records and taking appropriate action but ensuring senior nursing staff are kept informed at all times.

MAIN TASKS - PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. To undertake the management of care for a group of patients.
2. To assess the physical and psycho –social needs of patients and deliver therapeutic interventions based on best available evidence to improve health outcomes.

Professional responsibilities
• To adhere to the relevant professional standards and Code of Conduct and be conversant with the Scope of professional Practice.
• To act as supervisor within own profession for students on placement and rotational staff.
• Act as a resource for other members of the team being responsible for providing profession specific expertise.
• Critically evaluate and interpret evidence –based research to change and develop services and clinical practice
• To identify own clinical supervisor and attend supervision on a regular basis to continually develop own practice
Managerial duties and responsibilities
1. Contribute to own and others performance by maintaining evidence based clinical skills and other quality assurance mechanisms as part of the clinical governance of the Buckinghamshire healthcare NHS Trust.
2. Promote the resolution and management of conflict in accordance with the Trust procedures and ensure disputes and significant breaches of procedure and codes of practice are brought promptly to the attention of the clinical team lead.
3. Report any unresolved difficulties in team /patient relationships to the clinical team lead.
4. To be responsible for the standards and supervision of those team members who are delegated tasks and interventions.
5. Assess Clinical and environmental risk and report any accidents and clinical incidents.
6. Communicate symptoms and findings to other clinicians / agencies and services.
7. Provide a safe environment for the treatment of patients and protection of staff by ensuring effective use of equipment and materials in compliance with health and safety and other relevant policies
Managing patient care/clinical responsibilities
1. The post holder will work within local and national policies, protocols, and procedures as they relate to the safety and welfare of patients, visitors and staff.
2. The post holder will maintain clinical competency in relevant clinical skills, participate in professional policy making and the development of a multi-disciplinary model of care.
3. Maintain and update patient’s record recording clinical activity in accordance with trust policy.
4. Collect, collate, and evaluate patient information maintaining accurate records related to assessment and care co-ordination.
